Social prescribing in Norddjurs Municipality – connecting care and community  

Approach
The project aims to test a new welfare model that promotes well-being, reduces health inequality, and eases pressure on the healthcare system. It bridges professional knowledge with everyday life by connecting the health sector to local resources.

The initiative links general practice with civil society through social navigators who support citizens with complex social or mental health challenges in accessing communities and non-medical services.
